Does anyone can explain me what are the next steps after the MR?

Inland Application.
I didn't pay upfront the PR fees.

I would like to know which are the next steps?
medical passed?. PPR? o somenthing else?

Medical Passed will be your next step. Your Eligibility, Criminality and Security are the next steps. You may or may not get Pre-arrival letter once you pass your eligibility (I have seen some people getting PpR without this) and final step is PPR

You got medical request on May 2019 right not May 2018?? You should receive MEdical Passed within a month

One more thing I would suggest you to pay PR fees without even asking them unless it will delay your application for few days unnecessarily
