Submitted application on 13th Jan 2018 a msg from cic and when I open portal it says your action is required . What does it mean?

1. We received your application for permanent residence on August 15, 2016.
2. We sent you correspondence acknowledging receipt of your application(s) on December 5, 2016.
3. We started processing your application on December 5, 2016.
4. Medical results have been received.
