I rceived my AOR today

A question

In the london visa office specific requirements theres a form included IMM5562 (i did not fill this form while sending my application to CIO), shall i have to fill this form and send with my full application to LOndon office

As far as I know, visa specific requirements, need only be filled in for the visa office (not for CIO).
So if the form pertains to you, you should fill it.

BTW, why is this urgent. You should have 120 days to do this. You do know that right.

Imm 5562 is supplementary information form in london Uk visa office form....its a travel history form.

its a newly added form in the checklist, and yes you have to fill it.
