Please need help regarding question on the citizenship form

Hi ,Can somebody please help me with Question
14 A) Have you held travel document and or passports during your eligibility period

IF yes then fill up table A and if No then fill up table B

In my case I held travel document which is lost and I always had passport with me so what should I put in Table A

So my question is if I have to mention about my passport as well as Travel document in Table A or

I shall only put info about passport in the table A and in table B I should mention that I have lost travel document.Can somebody please help?
