PCC from Indian Consulate Chicago - Time taken?

Hi @pk46 , Did you send your documents via mail to Chicago Embassy? Did you get your PCC?
I'm in the process of sending documents to Chicago embassy via mail. But, I'm little confused about personal particulars form.
They're asking for dates for which I need police clearance in the top part. Did you fill this section?

Hi Mohan. I have replied to you in the other thread. ,

I visited Chicago Indian Consulate and got PCC the same day. And the "Personal Particulars Form" is a new addition, it did not exist till February 2018.

I did send my documents to Chicago consulate and I didn't fill the top section.
I left it blank and sent it to them.

I too think its for consulate to fill. Have you received the PCC yet?

Yes, I received it.
It took 2 weeks to receive it after mailing my documents to Consulate.

What kind of shipping you have used? Did you guys use USPS Priority mail or Overnight?
