I am trying to fill the FD-258 Fingerprint Form so I can get myself fingerprinted and send to same along with application form to the FBI requesting PCC for my response to ITA for Canada PR.
Can someone tell me what I need to fill in the field 'Employer Name and Address' on the FD-258 Fingerprint form ?
Also, Please suggest do we need to send a Covering Letter to the FBI along with Application Form and FD-258 Fingerprint form. If yes, what should be the contents of this Covering Letter ?
