Passport surename different from permanent residence

Hello po newbie here my tanong po sana ako balak ko sana mag apply ng conjugal sponsorship pero confused po ako kung ano gagamitin kong surename as my Philippine passport eh gamit ko ang ex husband surename nia, & sa permanent residence ko hindi ko pa inupdate before ung time sana na i update ko pr ko pero nag ka problema kami at nauwi sa divorced, nagrenew ako last year 2021 at nag stay ako sa single surename ko., mag aapply ako ng conjugal sponsorship ano po ba ang gagamitin kong surename?
