She has just received an email from Manilla office asking her to fill a form called Appendix A (joined with CIC email), and at the bottom of the form it is written Passport Request.

Is this the passport request step? Do she has to send her passport? We didn't see any specific instruction to send the passport to the Manilla office, they wrote in an attach letter we have 45 days to send the appendix A to them.
As your wife is from a visa-exempt country you probably won't have to send it the passport at all. They will process your file and sent the COPR to the mailing address when it's completed.
You can send a copy of your passport with the Appendix A if you want. Be aware that Manila asks for the passport quite early in the process, so it doesn't necessarily mean that the COPR will be issued right away. However, it's a positive thing for sure - somebody is working on your file.
