Only new passports should be sent, however you can do a cover note to inform your VO that the previous passports have expired and attach a copy of the old passports.
That was what i did in my case and everything went well.

1. Should I attach photocopies of birth certificate & passport pages certified by Notary Public or without certification will serve the purpose
2. Under column 13 of checklist (Identity and civil status documents). Birth certificate is sufficient or National IDs are to be sent?
3. Fees of CAN$ 550 is ok or medical fees is to be paid
4. Supplementary information IMM 5562- I travelled only Canada for study/work purpose. Should I fill this form also?

Your file will be returned if u fail to include Supplementary info IMM 5562 "your travel". It is a must for every1 to fill even if u have never traveled out of ur country before. why not download application guide or follow the instruction on the checklist so as not to make mistake. Goodluck
One more question about birth certificate & National ID. These certificates are in both language i.e. English & Punjabi (native language). Should I get it notorized or the only photocopies can serve the purpose.
For Identity and civil status documents, are both documents i.e. birth certificate & national ID sufficient? please confirm
