Option-C Printout

So i called revenue canada to send me a option c printout. I got a letter in the mail that looks almost identical to my Notice of Assessment with some added info. But no where on the document it saids 'OPTION C'.
Is this the option c printout CIC wants me to send to them? It shows my Marital Status, charitable donations and gifts, foreign properties, etc....has much more info than my notice of assessment.

I think there's a number of some kind at the top or bottom. I can't remember what it is, but what you've described sounds like the Option C. Gotta love how the government asks for a certain document, but then that document doesn't show what they're asking for...and around we go...

I took a look at my option C print-out, and it looks as you described.
On top is says "Income tax return information regular" and it contain info that you mentioned such as your address, SIN, marital stutus, date of assessment (on top)... it also include total earning, investment , RRSP deduction, and so on.
You have the right document.
