An Option C is NOT a T4. T4 you can get from your employer by a simple request or even CRA. However, the Option C is a complete different paper. It is more of a statement that shows more information regarding your living status...
Whether you're on welfare or any other social services. It has more details that tells CIC on whether you are getting financial support from the government or not.
Yes, you must call CRA for it. Be prepared to answer personal questions like your SIN number, DOB, Address and most importantly, a piece of information that is answered on your T4.
Note: a lot of people got confused about the fact that it doesn't say "Option C" on the paper. IT DOES NOT.

If you don't have an account, you may still be able to set one up as long as you can answer certain questions about your prior tax returns and notices of assessment,
When you log in you will see a "Canada Revenue Agency" page with tabs across the top and quick links down the left side. The first page shows your most recent information - mine shows my most recent Notice of Assessment. Do not open that. Follow these instructions:
1. Go to the second tab called "Tax Returns"
2. Click on the top link entitled: "View Returns and Notice of Assessments ...."
3. The first column shows the Year; the second the Status. On the Status column, click the most recent "Assessed" You will see your detailed tax return with all the line numbers. This is your "option C".
4. On the top right there is a link for "printer -friendly version". Click the link. On the top right again is a link for "Print"
I have the ability to print to Adobe PDF but not everyone will have this. If you do, you have a nice red and black document with your "option C"
I requested this by calling into the Government. I was able to just enter the info via phone and request it....and I got this really quick, I think it was like 5 business days (Ontario to Alberta).
