Open work permit with spousal sponsorship application

Hello,

On the government website it says that the open work permit (OWP) needs to be submitted online whereas in the spousal sponsorship application instructions it asks to include the OWP application together with the sponsorship application. I cannot find paper version form for the OWP.

How do I submit the OWP application with the sponsorship application?
