Open work permit extension

Hi all,

I am in the process of applying for extension of my study permit and my wife's open work permit. She is currently employed as Pharmacy assistant at London drugs. I have few questions in filling IMM5710 form.

In page 3 of 5 of IMM5710 form "DETAILS OF INTENDED WORK IN CANADA" section
1a. WHAT TYPE OF WORK PERMIT ARE YOU APPLYING FOR? I select "OPEN WORK PERMIT"
From Question 2 to 7, can I leave these sections blank as open work permit is a non-specific work permit?, even though she is currently employed and will be working for the same organization once her work permit is renewed

Also, if I am right, in the employment section (page 3 of 5) I need to fill her current employment status (from: her start date of employment, To: Work permit expiration date)

My study permit extension question:

I had to undergo administrative processing for my US visa (Visa was issued). Should I mentions this as visa refusal in my IMM5709 form??
