No, it doesn't contain names and passport numbers of dependents.
It just contains the main applicant's name and current address, date of birth, nomination certificate number and issue and expiry dates, EE profile number, job seeker validation code and NOC.

you can take money from your father/mother as a gift deed and create an Affidavit for it stating account numbers , ID card numbers and all other relative stuff, dont forget to get it Notarized

1. Affidavit of Gift Deed;
2. Attested Letter of Change of Name for a previous company;
3. Affidavit for no name in the Birth Certificate.

2. Attested Letter of Change of Name for a previous company; - Work History, with the Reference letter for that company. You can put a page separator for the reference letter and the change of company name.
