I was filling the PDF format application form for OINP and knew that all the fields which are not applicable to me should not be left blank and N/A should be mentioned in all such fields. However, when i was trying to put N/A in some fields like in the dates section, the form didnt allow me to put N/A. Due to this reason, i just mentioned an ambiguous date and explained this problem in my cover letter sent with the application .
Will this cause any problems with my application and can my application be rejected because of this?

sorry to say but your application will definitely be rejected and sent back to you.
for places where you cannot mention N/A while filling it in your computer, you leave those places empty, print out the form and then write N/A by hand in them.
what do you need to submit for "Evidence of good academic standing and length of program(s) completed (spouse or common-law partner only)". and is it mandatory to submit it?
I am planning to submit
1. Final Degree certificate
2. Consolidated Grades Marks sheet.
3. IELTS Score sheet (Took it so, placing along with others)
4. ECA Report (which helps for conversion)
I might be wrong but who have applied may correct it. By the way where are you planning to place DD in the application form? Is it to the second page of the application form?
