What do you input in Occupation, address of employer and employer contact number (Online OCI Form) and Professional detail/Employment letter in case for UNEMPLOYED applicant ?

Hi BLS sent my OCI application back because the name on my Canadian passport is different than that on my Indian passport. My middle name is there on my Indian passport and Candian does not have a middle name.. What document should I submit to resolve this... They are asking me to get the affidavit. Does anyone have any experience with this kind of situation?

discuss with notary your requirement, they will draft the affidavit for you, if you are ok with the wording go and get the affidavit certified with the notary

Just wanted to clarify that you do not need to courier any original documents to BLS for OCI application. You just need to write on the photocopy "This is a true copy of the original". Sign and date it. No need to notarize it either. If you want to confirm, call BLS at 289 498 1320
