In my case, I did upfront medicals. I downloaded the form given by the doctor in the list of the documents in my account.
I guess you need to send the form too.
Maybe someone who did medicals after AOR can confirm.

As I did upfront medicals, I download it before submitting my application, but in the case where the medicals were done after AOR, in my opinion the correct way would be to raise a webform in IRCC website to inform them that you did the medical exam.
I hope someone with no upfront medicals and same case would explain better !

If the printout says "Information Medical Sheet" - that's what you need to scan and upload.

So I gave my biometrics and they don't ask for the forms, i don't know why it was written on the appointement letter.
I have another question, do i need to send to ircc saying that I gave my bio or IRCC automatically knows ?
