You have to accept it on mycic account, start filling the online questionnaire, pls use fsw documents checklist as a guide to upload all your documents, you will have to do upfront medical. Thanks, you can pm me.

The form will be given to you by the Hospital after you have done your medical. just pm for more info. Regards
