I have misplaced my 2014 Notice of Assessment. Any advice as what I should do? Should I request a copy from CRA or get a printout from my online account?

I would just pull it from the online account if you have access.

Yes. CIC will get all the info itself from CRA. We do not have to send this document. We only send whats written in the requirements.

Neither the application form or the instruction guide requires to send such documents.

NOT need to submit NoA. The whole point of you giving consent is that they can ask CRA directly.
It is completely pointless (and in the worst case: annoying for the case worker) to include NoA in your application.
It really wouldn't surprise me to know that applications get held up when submitted with extraneous documentation. When dealing with any government agency, I only submit what is asked of me.
