yes you are the applicant and your husband can sign on the right side, where it says, spouse
in the form in need to fill out my husband details right?
whr it says name and everything uci no. and everything? do i need to write mine or my husband?
i m already a permanent residence so i dont know who should i enter?

Hi,
After going through this form it appears that, you have to sign as applicant and your husband has to sign as spouse.But do mention your identification no and not of your husband.

I think, Your husband's signature and his UCI no will go in the form and you will sign as spouse and your info will go in 5 &6.
Reasons: 1) Your husband is the applicant in original application and he is in India so he also got a new UCI#, you want to inquire about status of his file, so you can be the designated person and your husband is applicant.
2) If you read point 2 in step 7 of declaration, it says " I authorize immigration dept to release my info to above mentioned person." So obviously your spouse is authorizing the designated person to collect info on his behalf.
