Need Help regarding Schedule A

I have received communication from CPC-Mississauga saying ... "You must complete the Schedule A Background/Declaration (IMM 5669) form. There is time gap in section 6" after submitting the Schedule A via IRCC webform. I was also asked to submit the form to CPC-Mississauga via mail or through the IRCC Web form. I will be sending the form from India through courier service. Now, my question is which address should I send it (as outland spousal applicant ):

Should I also need to give any heading on the top of the address like "Additional Documents Required" etc.
