Need help for ECA- urgent

Guys,

I have to get ECA for Indian degree and received transcripts from university.

Do I also need to get Academic Records Request Form http://www.wes.org/ca/required/International_Transcript_Request.pdf signed by university or transcript alone is enough. Getting this document sign is not easy task.
