One of my previous employers have refused to provide me the reference letters as per CIC guidelines. So, now I am planning to provide following documents as per suggestions found in this forum:
a. Standard format service letter from employer on letter head.
b. Letter from one ex-colleague stating my roles and responsibilities with company working hours and benefits mentioned in it. I will get it notarized as well.
c. Last 03 months pay slips.
d. Appointment Letter issued to me, when I joined the company.
Now, I want to add other letters such as my performance appraisal and promotion letters but the issue is that my name on those letters is including my surname which is not part of my official name (name on passport). For example, my official name is Mr. X Y but some of the letters mention my name as Mr. X Y Z (Z being my surname).
I missed this thing during the course of my employment and now the ex-HR is refusing to correct those letters as well.
Senior members; please advise, what should be my line of action in this matter ?
Further, item d i.e. my appointment letter also has different spelling of my father name e.g. as per my official documents,it should end with "ak" but it ends with "aq". Kindly advise, will it cause any problem ?

I personally think the 4 documents you want to submit should be sufficient. However, you can add the other documents and explain the name issue in you LOE and also sign a notarised one and the same person affidavit.
Here is a sample:
Can you please also share your opinion on this part of my previous post:

I'm preparing the job letters to prove 1 year of work experience in Canada, but I got in a specific situation: I worked at Sears Canada and now they're closing their doors.
Do you guys know how can I prove it? They have compromised to give me the letter, but what if the immigration call them when I get invited?
