My spouse changed her name after marriage. What are the documents required to be attached for the name change? Her current passport, National Identity Card and Police Certificates are based on the new name. Do I still need to attach any other legal document. I am just planning to attach a LOE alongwith the new passport and old passport (with old name on it) as a proof.
Notarized Affidavit is important. If you insist to submit the application , Another option is after submitting the application , you can upload this document later.
If you are in India ( in our state) the village office gives the one and the same person certificate. That would be better.

Maybe you can also upload an old passport bio page(if you have any) with your old name on it, I am doing the same thing... however I think those that you mentioned are enough....
