Mothers name different in birth certificate and Passport

Hi All,

My mothers name is different in my birth certifcate and my passport.

For eg.

Mothers name on my BC is Laksmi
Mothers name on Passport : Padma Lakshmi singh

Even the spelling of Lakshmi is different on both. My mother passed away last year. In this case what do i do. Her death certificate has her name Laksmi Singh.

Is it ok if my father gives me an affadavit stating that my mothers name on my passport is correct...if yes..can someone please share me the format
