Military Service form - anyone has it?

New delhi immigration office sent a request to my dad for submitting Military Service form, and the email says it is attached with the email that we receive. But there is no such attachment with the email, I wonder if anyone of you ever got such requests and have the attachments or any Form No that you can help me with?

I have replied back to those guys asking for the attachment, but would be quicker if you guys have the form no, and I can download it from CIC website or you can forward me as an email attachment, would really appreciate that.

No, nothing and this is exact wording;

The attached details of Military Service form to be completed by you and returned to this office.

But there is no attachment. And they gave my dad 30 days to submit this lol.... thats even weird.

Call the call centre monday, or submit a case specigic rnquiry (now). Advise them the form wasn't attached. Request a resend.

Done! Good Idea, I guess now I just wait for their reply....

Problem with IRCC and forms is they do change them. It's better to just get them to resend.

This guide has the military service form. http://www.cic.gc.ca/english/pdf/kits/guides/3911e.pdf It's usually used by applicants from the Middle East but I suppose it will be ok to use by your dad. Maybe call cic to confirm on Monday.
