I am a july 27th, 2019 applicant. My medical was passed in December and file was transferred to LVO in January.
When I ordered gcm notes, they state the following
‘An original upfront medical report form IMM1017B signed by panel physician or an E-medical Up-front medical notification informed sheet has not been provided’

What does this means and what should my next steps be? Please help

That statement appears to be correct, as you did a medical on request from IRCC and not before the application was submitted. It's probably just a historical statement.
