Two weeks ago I received confirmation in the email that the Detroit office will send me "mdecial forms and instructions" in the next few weeks. I still did not receive the forms. I did my medical on July 12 in Toronto. The DMP already has all medical forms. They only wrote down my file number and will send the result to Ottawa.

How could Ottawa give my medical info to the Detroit office? I did not see anything written about the visa office. Would there be a problem as I did my medical a few weeks in advance?

What do they mean by "instructions" that will be sent? When will they ask for the landing fee?

Could anyone please tell me?

Usually the visa office will send to you a medical request with form 1028 i.e section A having ur photo , name , file no and a instruction letter for RPRF AND a deadline date say 60 days. How can a DMP do a medical without seeing the Sec a part.
The next is section B to E. The forms is called IMM 5419. This is not send to the candidate as all Canadian Immigration DMP's have A CD given to them from where they remove copies of the same. Clarify this with ur DMP.

Re: Medical Request from Detroit Office

I received today my medical form Section A and instructions in the mail. Section A is for client identification and summary. The instruction sheet says Section B to E can be found at the DMP.

I did my medical (in advance) about two weeks ago in Toronto when I received confirmation by email from the Detroit office, following my interview in May 2010. The DMP already filled out information in Section B to E and will send the result to Ottawa probably next week.

What do I do with the medical form Section A (Summary)? Do I have to send it to Ottawa or my visa office in Detroit? Do I have to follow up with the Detroit office whether they receive my medical result?

Did anyone applying from Canada go through the same situation? Could you share your thoughts?

Check out with th.e DMP as u did ur medicals on 12th July. He will advice accordingly. Do not take any hasty decision from ur side at this stage.
