i called 2 different doctors for medical exam inquiry regarding my wife's PR application. one said she needs to bring 4 Canadian PR photos to the appointment, and the other doctor said she needs to bring 5 US passport photos to the appointment. and according to the document checklist in the application form, it just asks for 5 recent photos, not saying it has to be the formal passport/PR photos. it is so confusing.
1. anyone went thru this and able to tell what exactly did you provide?
2. also i was quoted between $400-450 total for the medical exam, sounds right?
3. the doctor office said they will mail the medical result to Ottawa or some addresses in Canada. I thought they suppose to mail it to Buffalo, right?
The photo requirement is listed in your region specific guide. I needed 5 passport type and I had a page showing the required dimensions (I don't remember where the page came from, sorry). The DMP will attach one photo to the receipt you receive after the exam which you will need to include in your application. The doctor will send your results to a regional medical center in Canada where they will remain until requested by the Buffalo office when your file starts processing.
I got my exam in Canada because there was no DMP near where I lived in the states. It only cost about $250 here, but I am fairly sure it costs more in the states.
