I did my upfront medical exam today and at the end, I was given a single page document by the doctor. How can I confirm it is the valid document that I need to upload?
It is not IMM 1017B Form , nor it is e-medical form.
Can someone explain what does the valid document look like? OR if someone did their test - what kinda document did they get?
Well, mine says - Client ID , UMI no. , Date, Full Name and my current address - It has doctor's stamp - that's it. There is no photo , passport number etc
Anyone else? My form title says - "To be Completed by Applicant/Examining Doctor and Included with Application"

It's only a single page printout. The only difference from above ones is that these are for Australia, and for IRCC, it would be mentioned IRCC/CIC instead of DIBP and it will have UMI written instead of HAP before the reference number (which starts with U). All you need is a UMI No. and a scan of this e-Medical information sheet to upload to your MyCIC account.
Hi everybody i have saved that e medical information sheet in my desktop of my dependents and sent through webform because there was no any placeholders in my CIC account,So my question is does it make any differences by scanning or without scaning.
