It needs a stamp and date from the photographer (irrespective of single or family application).
If it is a single application then you need not necessarily sign/write your name at the back.
However, if it is a family application, you have to write the name of the person at the back.

Stamp and date from the studio, and you have to write your name on the back of the photo, Make sure the exact specification from CIC website and photo must not be older than 6 months

I just have a question regarding Minor's application.
Shall I apply under Canadian citizenship - Minors (under 18 years of age) under subsection 5(2) for my child or under Canadian citizenship - Minors (under 18 years of age) under subsection 5(1) ?
Under section 5(2) its not mandatory for minor to complete 1095 days where as in Subsection 5(1) the child is being assessed independently after completion of 1095 days.
Does this have any long term impact on the citizenship of the child or not??? I mean Limit to citizenship by descent [subsection 3(3) up to first generation only?

Limit to citizenship by descent: its mostly same in all countries. All top countries have similar rule and restrict up to 1-2 generations. Your kid needs to plan and choose which one they want. You cant expect a country to issue free citizenship for ever even when one lives abroad for generations.
