Sa nationwide ako ngpamedical...Yung nauna sa akin alang orig passport, ang ginawa lng nila 2 copies/ more than the required copies ang binigay nila.... however, bring other IDs as suggested by OJ
Based on the MR letter sent by CEM, you (if you're the principal applicant) and your dependents (if any) should present your respective passports to DMP at the time of medical examination. Per my experience, the passport was used for identification or recognition purposes only. Ewhy is right, the DMP wants to make sure it is the real you who is being subjected to a medical test. Therefore, I think passport is not an absolute requirement for medical exam.. Any valid identification card should be acceptable in lieu of the lost passport. The DMP should not compel you to produce the passport if it is lost, considering the limited time given to you by CEM to complete the medical exam and the lengthy time it takes to apply for a renewal passport.
Just to be sure though, you should bring and present a government issued ID such as SSS, Driver's license, etc. instead of other ID (like the company ID). And your dependents (if any) must bring their passports, if available.
it's a must na ipresent mo yung passport mo, lalo pag sa x-ray na, kasi chinecheck nila kung ikaw nga talaga yung applicant... pero you might wanna ask the DMP kung san ka magpapamedical... Kami kasi nung magpamedical, di na tiningnan yung passport namin nung DMP, pero pagdating dun sa lab, hinanap yung passport ko kung pa-xray na ko... hope this helps...
