You can sponsor while on disability. There is no minimum income requirement. You and your spouse need to show that you can support yourselves in Canada without welfare (disability being an exception).

Well I plan on getting a part time job so that should help. Would it be wise to hire a representative to help this process? I keep reading about them.

You don't need a representative. Most people do this process themselves.

She can't change her name until she is actually married.

As she is American, she will need to change her name in the US, not in Canada.

Only if you apply inland.

Yes, both applicant and sponsor must be living inside Canada at the time of the application to apply inland. You can apply for an Open Work Permit as part of this application, which takes about 4 months to receive. Inland apps are generally processed slower than outland, especially outland US.
