Hi we are filing a "work permit only" application I had the below queries:

Form ESDC EMP5602 (2015-10-005) E (LABOUR MARKET IMPACT ASSESSMENT APPLICATION HIGH-WAGE AND LOW-WAGE POSITIONS)
Is this the right form?

Supporting document:
Letter from a legal business confirming the existence of a contract for a good and/or service with the employer applying for LMIA

What exactly does this document have to inlcude and from whom? the companies client?

Any help will be very much appreciated.
