legal document for 'name changes'

Hi

I am applying for Canadian PR. One of the required documents is "legal documents for any name changes".

My name on my previous passport is "X Y Z". About five years ago, I decided to modify my name, and I got a new CNIC and Passport with the name "X Y". The question is, what kind of legal document do I need to account for this name change?
