We need to know all of the names you have ever used in order to verify your identity. If you used any other names other than the one being requested in your grant of citizenship application, print them in the chart. Examples: Name at time of birth, name before marriage, previous married names, married name, nick names or any other names you have used. If you have legally changed your name, see Appendix A – Changing the name on your document.
My Birth certificate name is : AalapBhai
Name on passport / Canada PR : Shah (Family name) Alap Hareshbhai (Given Name)
Do i need to add my birth certificate name on the table ?
is this also consider under legally changed name > and Appendix A applicable in this case ??

i haven't change the name as name remain same, Only spelling has an extra "a" +bhai
during my SINP application, i made an affidavit which says both my birth name and passport name are same
so in this case where i have not change the name , Appendix A applicable ?
if i add my birth name in chart which is not matching my passport name or name i want to grant citizenship , any doc need to attached ?

If you haven't legally changed your name then my first guess would be that you don't need to submit further proof, but I don't have a clear answer on this as I don't know why one would use two different names in this manner. Hope someone else can help.
