I'm in the same situation (except it was a visa for USA. It was rejected around 10 years ago, and I've had 2 visas since then, with extensive travel.)

Should I raise a CSE?

I would raise a CSE, especially as the US and Canada share information

Usually BG 'in progress' is IP1 and 'we are processing' is IP2

Yup, it is. Do you currently reside in Canada or the US? That might be why you moved to ip2 so quickly

No. I am in India.
I wonder why then.. But my husband has 20 years valid uk and us visa, will that help??. There are people who stays in ip2 also for months. My aor is June 20'.

It takes 30-40 days for you to receive the notes, and they show what happened on your file till the day you ordered them.

Calling is a much less detailed way of getting the exact status of your file. Ordering GCMS notes is by far the best way.

That would be awesome!! Have a feeling that my case was moved to London Visa Office yesterday, am gonna order GCMS notes to see what's going on! It's great that changes are happening in my profile. How's yours going? Any movement?

I'm glad yours is moving! If you don't mind my asking, in your ADR, they just asked for the IMM5669 generically or stressed something specific within the form?

Just asked for it generically. It was weird too, because we had already provided one in our LEO. Perhaps they didn't see it or perhaps we filled it out wrong on the first try. What about yours, did they ask for it generically or did they specify something?

They specifically asked "Schedule A yada yada (...) Please include in the "Personal History" section the following periods: 2013/05 to 2015/10 and 2016/10 to Present. Do not leave any gaps in time." Those periods were before and after I attended a post-graduate course. I hadn't provided a Schedule A upfront, but I didn't think there was much more detail I could add. I also asked myself whether I had left anything out. It's odd.

What intrigues me is also that I received this ADR way before my MEP. Folks usually get ADRs when they're in IP2. I know each case has its own pace, but I don't know. It seems so random...
