Its online now a days and needs to be filled online , you dont need to download and fill explicitly and upload

Yes online i cant find the proper form, first one i got had a javascript error, second one doesnt have the option the validate button and the third only opens in my adobe acrobat reader and everywhere else it displays a “please wait” error
