Quick update on our situation: we received COPR today from Paris VO, barely 3 weeks after PPR and 2 after they received our passport copies (no need for passport themselves as we are from visa exempt countries). COPR valid until January 25, 2022, and medicals got automatically extended to the same date. They do mention that no extension is possible after that date, though, so we would forfeit our PR application if we haven't entered the country by then.

When medical exams expire they are sent for re-evaluation. An automatic extension is valid for 6 months, while new medical exams are valid for 1 year. Now, from my understanding, you may be asked for new medical exams because:
1) You particular medical situation requires new exams
2) They believe that your application will not be completed in the next few months
3) They believe that you will not be able to travel before 6 months
It is difficult to say what is your case, but new medical exams do not automatically mean 1 full year of waiting time.
Hello I wasn't asked to do medicals again and they were extended for a year as I could see on my GCMS notes, I also got to see that my application was approved the same day
@legalfalcon Hello hope you are doing fine, after some reading online and on this forum tell me if I got this right, I already got my application approved and medicals extended to 22 dec 2021 as per my GCMS notes, still haven't received the COPR nor the PPR mail, until I get them I can't travel to Canada due travel restrictions? Even if I have a brother there? And if I could rejoin Canada if I am exempt of travel restrictions can I do it before receiving the PR? I f so would it matter if my brother is actually living in Montreal and I am an express entry applicant?

If you fall under any travel exemption category, you can apply for a TRV and if you get an AL, you can travel to Canada. It does not matter where you stay. However, as a FSW you should not intend to live in Quebec.

Thank you for your response. Would it help going to get my PR sooner? We were supposed to go to Mississauga as there is a special facility to train foreign dentist for the board but I wouldn't mind going to my brothers in montreal to make things go faster, due to this pandemic many things were postponed.
For all applications, meds have to be valid when the application is being processed as well as when you land in Canada. If your meds are expiring or have expired, then they will be sent for reassessment and the Regional Medical Officer will make a determination on whether to extend or ask for re-meds.
Usually extensions are for 6 months. Given the current scenario where its difficult to predict how long the travel restrictions will stay, most applicants are getting re-med requests. After a re-med the medicals are valid for another year.

IRCC will request re-medical if required. You can get it done on your own accord, but if your spouse is non-accompanying, let IRCC decide if they need it or not.
They were automatically extended. They never informed us of this, on gckey or via response to the several webforms I raised about it. I just saw it on the COPR.
