I don’t think so. I submitted the Nomination Declaration form, first, then got ADR for Schedule A IMM5669 form and Additional Family form.

No single word about any forms. You don’t have to upload anything that system doesn’t require you to. You provide all the necessary information when answering the questionnaire.
If they need any additional form for some reason, they will send you an ADR.

However, paper based PNPs have to fill everything of course.

Send the Nomination form, and if you have any visa refusals, send the schedule a background declaration form as well. Just raise cse and explain that you would like to submit the two forms and attach them.

It really says you have to complete the form for PAPER BASED. I think if they need extra forms they will ask for them.

You are right, I had the same query and after referring the website, I dint submit Schedule 4 form with my LOE.

However, I read regarding this form in the forum, that CIC sends out ADR for filling up Schedule 4 form, hence I raised this query ? Well its up to us, if we want to send the extra document or wait for ADR.
