Hello , for surrender passport application....in applicant details part given name means only our name??? Bcz in my Indian passport my name is with my father's name..I am confused now.please let me know..

Fill the application with details as they are in passport. Also any other details fill them exactly as in those documents, for example, the address should be written word to word same as in the address proof. Same goes for every detail in the application. If there is even a single mistake they will reject application.
To answer your question, write your name along with your father's name, just as in passport
