I would put the name of your company (if there is a name). If there isn't, just put your own name or write N/A. I haven't found any examples of this online - these are purely my recommendations in case you don't hear from anyone here.

Self employment is employment (work). As such, it must be listed as "self-employed" in 6F. However, I would write a brief explanation as to what you were doing. For instance, if you were a subcontractor, include any contracts (photocopies) of the parties that contracted your services out. But DO NOT leave any time unaccounted for.
You would need to put what school or work you were doing/attending. If self employed you need to put self- employed - There is no employer name necessary if your self employed. Put what you were doing/attending during the period they asked. Make sure you attached all relevant documents relating to your self employed jobs.
CIC call centre reps will say anything as they do not know (one will tell u something and another tells you a different thing. I suggest you go to one of the YMCA who has experience in these things. That's where i did my application and I was not asked for one piece of additional document.
