Self Employed What will I write ??


Should I write my name ?

Exactly; you have to put the name and address of your incorporated (or family business). The company that you were on payroll.
For ex, I was myself self-employed for 3 out 4 years, and for Employer I wrote: xxxxxx Ontario Ltd. with my home address as the company address was my home address.

None. I havent attach any additional documents, but of course I had ready to send right away, my incorporate tax filling/assessments, HST return, payroll statements, business bank statements, work contracts, etc.
I was not asked at any time during process to provide any additional document.

But look; CRA/CIC is same government, they always can check by themselves and I would be surprised they didnt.
