IMM5708 question

I know I know... There's a "visitor" forum but I've asked on there with no answer.. Hoping one of you kind peeps will help a girl out..

Question when filling out this (what im sure should be simple!) form to submit my restoration of status application online:

- If i am out of status should i put my status as “other” then write “out of status requires restoration” in the blank field? Then leave the date section blank? Or do i put a date? I read that for a form on my PR app but now i cant find where i read it.

Yes, put "Other" and "Out of Status". I would put the date that you lost your status and then write present. Honestly, you are stating at the top of the form that it is a Restoration app and they will know you are out of status anyways, so it's not really going to matter what you put.
