And for deceased parent. Its asking city country and date??? There is already dob column. What other date and place are they asking?
Place where they passed away and date when the passed away????

It says "if deceased: give city/town, country or territory and date."

That's pretty self-explanatory: write the city and country where they passed away, and the date.
