Check the box "yess" or "NO" to indicate if you have ever:
1.applied for any Canadian visa(s) either as a Permanent Resident, Temporary Resident (Visitor, Student, or Worker), or a Temporary Resident Permit.
2.been refused any kind of visa to travel to Canada. When I was like 15 years old, my mother applyed for a temp visitor for me and her. I didn't even know as I was a minor. She listed me as a dependant or whatever, not sure what it's called. Anyway, I got the temp visitor visa back then but I never used it since I lacked interest to visit Canada as I was having fun at home and school. Do I have to mention this?

1. Where did you find this question? I can't find it on the IMM1294E form. Anyway, I believe it should be "YES" since your mom applied for you too. You can explain the details section that it was your mom that applied and you were a minor.

2. the answer would be "NO" since you and your mother received the temp visitor visa.
