IMM 5669 back ground declaration !

Anyone plez help me understand this question in the form IMM 5669
Question 6- Have you, or, if you are the principal applicant , any of your family members listed in your application for permanent residence in Canada ever


(D) been refused refugee status , an immigrant or permanent resident visa ( including a certificat de selection du Quebec or applications to the provincial nominee programme ) or visitor or temporary resident visa , to Canada or any other country ? Yes/No
(E) been refused admission to, or ordered to leave Canada or any other country ? Yes or No

Background - I am the only applicant for PR ( inland spousal ) no kids or dependents
And my tourist visa to US was denied in 2012. So do I need to answer Yes and explain or ?? Or this question is for the applicants family members ??

We answered yes to D, and then explained in the box. We ticked no to E as we were not refused entry, since we never got to the border. Some people tick yes for both and then explain.

I just want to ask regarding IMM 5669,

Do we need to validate this form? Because I don't see a validation button...

Please help....
