We were recently sent our medical requests for our PR application along with an IMM 5409 to confirm our common law status. I am the applicants partner and am a bit confused if we do indeed need to complete this as it wasn't detailed on the letter with medical info.
Can anybody give any advice with regards to completing this form and who 'Commissioner of Oaths' are as we need to get one to sign the declaration.

I typed in GOOGLE 'commissioner of oaths NORFOLK' as a guess, and all sorts of names/addresses came up. Don't know details about completing your form, but hope this helps somewhat. Can think of no reason why VO would send you a document if they didn't require it to be filled out. If you are living common law and the form is to swear under oath that is your status, I would make every effort to complete and return it whether or not it was listed in the letter with medical forms. You must be happy to be this close to the end of this long journey. Good luck.
