RE: STATUTORY DECLARATION OF COMMON-LAW UNION form

Hi!

Quick question. We're filling out our application for my partner's ITA and found this common-law form in our checklist.

Q1: It mentions reading the instruction guide before filling it in - I can't find this anywhere! Does anyone know where it is?

Q2: Person who witnesses declaration: A Notary Public, Commissioner of Oaths Commissioner of Taking Affidavits - in the UK, is this a solicitor as never heard of a Commissioner of Oaths??!
