Note 1 states that I need to sign if no spouse or common law partner is listed ... but then the signature states that "I certify that I do not have a spouse or common law partner, ex-spouse, or former common-law parter".
Consfused ... they questions / statements conflict - do I sign as I don't have a current spouse or common-law partner?

Hilarious that they still haven't changed this form... (Sorry, not funny, but it was so many years ago that I was trying to figure out what to do...)
I don't know what the best solution is, I can only tell you what I did.
I did not sign the form, instead I wrote "see attached explanation."
So on the attached sheet, I explained that while I did not have a current spouse, I did have a former spouse, and that was the reason I could not sign the form. Then I basically wrote my own statement (edited the statement to fit my situation) and signed it. I also provided my ex-spouse's name and address.
